New Patient Registration

Steps to New Patient Registration

Step 1

Check you are within the practice's catchment area.

Each practice has a catchment area within which they can accept new patients. If you are unsure whether you can register please contact the practice.

Proof of ID and address is also required e.g. passport/driving licence and a utility bill confirming your current address.

Step 2

Complete Registration Form

Please pick up a New Patient Registration form from reception or download one from the above “Registering Online” tab.

Please note - the new patient registration form is a legal document and needs to be

  • filled in,
  • signed
  • and handed back to reception

Step 3

Placed on the Practice list

Once you have completed and signed the form hand it into reception to be registered with the practice.

As a new patient you will qualify for a new patient check with a nurse. If you would like to take up this offer, please ask at reception.

Change of Personal Details

It is really important to us that we know how to contact you for a whole range of reasons - from simply changing an appointment to notifying you of an important test result. So please keep us informed if you move, or change your name or phone number.
