Steps to New Patient Registration

Due to increasing numbers of patients registering with the practice, please allow up to 7 working days for new registrations to be processed.

Step 1

Check you are within the practice's catchment area.

Each practice has a catchment area within which they can accept new patients. If you are unsure whether you can register please contact the practice.

Proof of ID and address is also required e.g. passport/driving licence and a utility bill confirming your current address.

Step 2

Complete the following registration forms:

Online Medical Questionnaire For New Patients

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Step 3

Placed on the Practice list

You will be able to speak to a doctor and book appointments at this point however, when you attend the surgery for the first time you will be asked to sign the form and provide proof of ID and address e.g. passport/driving licence and a utility bill confirming your current address, to complete the process.

As a new patient you will qualify for a new patient check with a nurse. If you would like to take up this offer, please ask at reception.

Change of Personal Details

It is really important to us that we know how to contact you for a whole range of reasons - from simply changing an appointment to notifying you of an important test result. So please keep us informed if you move, or change your name or phone number.
